Freeport, Illinois — The merger of Harmony/WellCare with the Meridian Health Plan brings good news for Harmony Medicaid plan participants in the area: Effective Jan. 1, 2019, all patients enrolled under Harmony will become Meridian Health Plan patients.
FHN is contracted with Meridian to provide care for Medicaid patients.
Harmony patients will receive notification of the change in the mail, and will begin receiving Meridian-branded materials on Jan. 1. No action is required from participants for this change.
